1. MEDICAID ELIGIBLE
If you are a Medicaid eligible recipient, approved by the state to receive services and would like to choose Capital Home Care as your provider for personal care services. In Two Simple Steps, you can start to receive dependable services taking a load of your shoulders.

Step 2: Call Your Support Planner:
After discussing your needs with you, we will confirm that we are able to serve you satisfactorily and the second step is for you to inform your support planner to include “Capital Home Care” in your Plan of Service (POS) as your personal care provider.
2. MEDICAID ELIGIBLE – Have not applied to receive services
If you are a Medicaid eligible recipient, and need the personal care services, but do not know how and where to go! Just take one Step.

We will guide you step by step towards the right resources and forward your information to ensure that you are able to save time as well get the necessary hours that you qualify for.
3. NOT SURE IF MEDICAID ELIGIBLE
If you would like to find out whether or not you could be Medicaid eligible and receive services through the state. Please visit: www.marylandhealthconnection.gov and you can apply to determine if you are Medicaid eligible. Feel free to contact us for any questions you have at any stage of eligibility determination and we will do our best to guide you towards appropriate resources.
4. MEDICAID INELIGIBLE but have Insurance:

Depending upon your insurance plan as well as the insurer, the terms and processes may be different. We will identify any and all specific needs to ensure that the set up and delivery of your services is smooth as we have done for so many clients. 5. PRIVATE PAY
For individuals that are not Medicaid eligible and do not have long term care insurance plans, a qualified caregiver can be referred and the services rendered can be paid privately.
